Appointments

Patients are seen by appointments only. It is our goal to see you on time, every time. Some appointments will conflict with school schedule. We offer both early and late appointment (8 am-5 pm) to best accommodate your time. We want to work with you around your school and work hours as much as possible. Your time is extremely important to us and we ask that you extend the same courtesy for us. If you are unable to keep an appointment, please notify us immediately to reschedule.

Emergencies

In an event of an emergency, please call us prior to coming so that we may properly prepare for your arrival. If any emergency arises outside of normal office hours, Dr. Dau may be called at home. We will do everything possible to keep you happy and comfortable.

Financial & Office Policies

Cost

The cost of treatment will depend on the nature and severity of your orthodontic problems. We will discuss with you the fees and payment options prior to treatment. There are several payment plans to best suit different budgets. We accept assignment of insurance from several insurance plans, and will file the necessary papers to the insurance company.

Payment Plans

We offer a variety of payment options to best suit your needs and have established the following:

• 7 % discount is offered for pre-payment of treatment in full.
• 4% credit card discount is offered for pre-payment of treatment in full.
• In-House Financing: a 10% down payment with direct draft payments out of a checking account over a period of the treatment time. A 30% down payment is required without automatic payment.
• For your convenience, we accept payments by credit cards, debit, cash, or check.
• We understand that there patients may encounter personally difficulties. As a result, we will make every effort to accommodate and work with such patients. Please do not hesitate to discuss with us any matter relating to treatment.

Insurance

Our office is committed to helping you maximize your insurance benefits. We accept most insurance and will accept assignment of insurance. Your insurance benefits usually cover a portion of the fee and you are responsible the remaining portion. We can only estimate your coverage in good faith, but cannot guarantee coverage due the complexities of insurance contracts. Any estimated insurance benefits not paid by the insurance company are the patient's responsibility.

Assignment of Benefits

To better assist you, we accepted assignment of benefits from most insurance company, which means that we will accept the benefit payments directly from the insurance company on your behalf. We understand that filling insurance can be a burden to your family. If we are unable to accept assignment of payments from your insurance company, we will more than happy to help file the claim for you so that you can be reimbursed directly.
